I can confirm that any of the above listed items have been thoroughly cleaned and decontaminated prior to returning to Velocity Safety Ltd.

I also confirm that the items have not been in contact or used with any hazardous substances previously including substances listed within EH40, asbestos, lead, micro organisms, nano technology, radio active substances or similar.

I acknowledge that I have a duty to highlight what substances the equipment may have been used with or in contact during their normal use or operation.

Velocity Safety Ltd reserves the right to investigate suspicious contamination by any means necessary and if proven the costs and damages will be forwarded to the customer who has initiated the equipment return request. This is regardless of if the contamination was known or unknown.

If the equipment history is unknown or questionable in any case by the user/owner then the equipment must be sent for specialist decontamination (where possible) prior to returning to Velocity Safety Ltd.
